Hazard Report Form

Use this form if you identify a hazard.

A hazard is a situation which presents the potential for an incident, accident or near miss. 

Do not use this form to report a near miss, incident or injury. Instead fill in the Incident Report Form.

 

Incident Report Form

Use this form in the event of a worker witnessing/being involved in any non-conformance or an incident resulting (or potentially resulting) in an injury or an unsafe practice or a near hit.

Fire Warden Training / Refreshing

Being trained as a Unite Church Fire Warden is important to helping to keep our church family and guests safe in the event of an emergency. You will need to refresh your training regularly.

This is simple - it simply requires refreshing you on what you need to know/do as a Fire Warden at Unite Church, and then complete a quick test (open book - use the content below) to show you are still competent.
